Service design for Care@home: promoting health literacy of patients, family members and specialist staff
There are numerous Care@home initiatives running independently of each other in Switzerland. Their aim is to facilitate cost-efficient care at home and reduce the load on the hospital infrastructure for the long term. The interactions between the various service components being planned are highlighted in a visual situation analysis for stakeholders. The project lays the groundwork for a subsequent project – the development of a learning platform.
The project involves the creation of a service blueprint for the Swiss Care@home initiative. The aim is to create a standard corpus of knowledge relating to all aspects of the interplay among the various service components (persons, tools, processes) in the Care@home area, drawing on accessible information and best practices. This should increase both understanding and acceptance of the Care@home initiatives among all stakeholders. In a subsequent project, a Care@home learning platform will be developed with the aims of minimising uncertainties on the part of patients, optimising the further training of specialist staff and building confidence in new forms of treatment.
The project encompasses the analysis of existing processes and of the informational needs of stakeholders, the development of a service blueprint for the communication of information in Care@home projects, the development of an initial plan for the realisation of the learning platform and the preparation of a project report and the application for the project to follow.
The methodological approach encompasses literature research, interviews, evaluation, design, conceptual planning and follow-up.
